Authorization and Waiver for Publicity Release For Minors

This release allows Sundog Theatre, Inc., their photographers, staff and representatives, the irrevocable right to use and reproduce my child’s name, performance, voice, conversation and sounds, photograph or likeness, in all forms of media for all purposes including broadcast on television, cable, theaters, radio, print, publicity, and promotion of every description in connection with Sundog Theatre’s programs in which I am participating or have participated. I waive my right to inspect or approve finished versions, including written or spoken copy that may be created in connection with it. No claim of any kind will be made by me. No representation has been made to me.
